Step 1
Complete the contracting paperwork for Bravo Health and fax it back to us at (817) 306-2357.
Step 2
After you have submitted the contracting paperwork to us and cleared the background check we will send you a temporary agent id number. You will need this number to complete the training portion of your appointment with Bravo. You will have two choices for fullfilling the training portion.
Option 1 is to attend a Face to Face training session in the local market with Bravo. If you wish to do this then you need to call us to register for a Face to Face session. Shannon Culp (877) 437-5010 extension # 2575.
Option 2 is to complete the training online.
Step 3
Fax us proof of completing the training portion in Step 2 and let us know if you have interest in our lead programs. Finally we will notify you of your permanent agent id number and you can order supplies online at
