announce the launch of the 2009 Care Improvement Plus Benefits Training
Program.
In order to sell on behalf of Care Improvement Plus for 2009, you must
first complete the Credentialing process.
In order to complete credentialing, please access the Credentialing URL
provided below and submit the three (3) required forms:
Acknowledgement and Authorization for Consumer Reports Ethics
Acknowledgement Agent Agreement
Credentialing

Client Package Code: [Empower2009] (You will be required
to enter this Client Package Code during the login process.)
After the credentialing process is complete, you will be sent an Online
Training URL to register and complete the 2009 Care Improvement Plus Online
Training Course. Once Certified with Care Improvement Plus, you will
be issued an Agent ID#.
