From Wellcare: New PFFS Health Club Reimbursement Form
Palladian Health has issued an updated fitness reimbursement form. Eligible PFFS members need to use this form when submitting a request for health club reimbursement.
- The new form has been issued in both English and Spanish.
- This form must be submitted to Palladian via mail.
- It may not be faxed.

CCP members who receive benefits through SilverSneakers® are not eligible for health club reimbursement. CCP members eligible for health club benefits must enroll in a participating SilverSneakers® program to participate.
